This study explored perceived benefits and risks of participation in HIV research among 33 female sex workers in Tecún Umán, Guatemala. Stigma associated with sex work and HIV was a critical barrier to research participation. Key benefits of participation included access to HIV/sti prevention and testing, as well as positive and trusting relationships between sex workers and research teams. Control exerted by managers had mixed influences on perceived research risks and benefits. Results underscore the critical need for HIV investigators to (...) develop population-tailored procedures to reduce stigma; engage managers; and reinforce trusting, reciprocal relationships between sex work communities and researchers. As sharing and secondary research use of biospecimens increases, IRBs and researchers face the challenge of protecting and respecting donors without comprehensive regulations addressing the human subject protection issues posed by biobanking. Variation in IRB biobanking policies about these issues has not been well documented.

Rare genetic diseases collectively impact millions of individuals in the United States. These patients and their families share many challenges including delayed diagnosis, lack of knowledgeable providers, and limited economic incentives to develop new therapies for small patient groups. As such, rare disease patients and families often must rely on advocacy, including both self-advocacy to access clinical care and public advocacy to advance research. However, these demands raise serious concerns for equity, as both care and research for a given disease (...) can depend on the education, financial resources, and social capital available to the patients in a given community. In this article, we utilize three case examples to illustrate ethical challenges at the intersection of rare diseases, advocacy and justice, including how reliance on advocacy in rare disease may drive unintended consequences for equity. We conclude with a discussion of opportunities for diverse stakeholders to begin to address these challenges. (shrink)

The metabolic organization of both normal and malignant prostate cellular phenotypes involves some unusual and surprising features. In particular, both conditions exhibit ratios of NADH/NAD+ and NADPH/NADP+ charactersitic of high oxidative states despite a chronic shortage of O2 in both conditions. In this paper, we observe that, in prostate cancer cells, the oxidizing power of the fatty acid synthesis (FAS) pathway is so large that redox is stabilized more favorably (more oxidized) than in normal prostate cells. This FAS‐facilitated redox improvement (...) occurs despite the fact that malignant cells are more O2 limited and therefore express more hypoxia inducible factor 1 (HIF1) and express hypoxia‐regulated genes more robustly. This unusual metabolic situation clearly separates direct regulatory effects of redox balance from secondary effects of hypoxia per se. The physiological significance of the FAS pathway is thus the harnessing of its oxidizing power for improving redox balance despite conditions of more extreme hypoxia. Similar hypoxia defense strategies are found in animal species that are unusually tolerant to oxygen lack. Our hypothesis is that the metabolic organization in the “low zinc, low citrate” phenotype reflects an hypoxia‐defense adaptation geared toward redox balance, with prostate cancer cells being relatively more oxidized, even if more hypoxic, than normal prostate cells. Recognition and understanding of these redox balancing and hypoxia defense functions may lead to new intervention strategies by developing new intracellular targets for prostate cancer therapy. Background Policy decisions about childhood vaccination require consideration of multiple, sometimes conflicting, public health and ethical imperatives. Examples of these decisions are whether vaccination should be mandatory and, if so, whether to allow for non-medical exemptions. In this article we argue that these policy decisions go beyond typical public health mandates and therefore require democratic input.Methods We report on the design, implementation, and results of a deliberative public forum convened over four days in Ontario, Canada, on the topic of childhood (...) vaccination.Results 25 participants completed all four days of deliberation and collectively developed 20 policy recommendations on issues relating to mandatory vaccinations and exemptions, communication about vaccines and vaccination, and AEFI (adverse events following immunization) compensation and reporting. Notable recommendations include unanimous support for mandatory childhood vaccination in Ontario, the need for broad educational communication about vaccination, and the development of a no-fault compensation scheme for AEFIs. There was persistent disagreement among deliberants about the form of exemptions from vaccination (conscience, religious beliefs) that should be permissible, as well as appropriate consequences if parents do not vaccinate their children.Conclusions We conclude that conducting deliberative democratic processes on topics that are polarizing and controversial is viable and should be further developed and implemented to support democratically legitimate and trustworthy policy about childhood vaccination. Phonological rules create alternations in the phonetic realizations of related words. These rules must be learned by infants in order to identify the phonological inventory, the morphological structure, and the lexicon of a language. Recent work proposes a computational model for the learning of one kind of phonological alternation, allophony . This paper extends the model to account for learning of a broader set of phonological alternations and the formalization of these alternations as general rules. In Experiment 1, we apply (...) the original model to new data in Dutch and demonstrate its limitations in learning nonallophonic rules. In Experiment 2, we extend the model to allow it to learn general rules for alternations that apply to a class of segments. In Experiment 3, the model is further extended to allow for generalization by context; we argue that this generalization must be constrained by linguistic principles. (shrink)

This study described the relationships between academic class and student moral sensitivity and reasoning and between curriculum design components for ethics education and student moral sensitivity and reasoning. The data were collected from freshman (n = 506) and senior students (n = 440) in eight baccalaureate nursing programs in South Korea by survey; the survey consisted of the Korean Moral Sensitivity Questionnaire and the Korean Defining Issues Test. The results showed that moral sensitivity scores in patient-oriented care and conflict were (...) higher in senior students than in freshman students. Furthermore, more hours of ethics content were associated with higher principled thinking scores of senior students. Nursing education in South Korea may have an impact on developing student moral sensitivity. Planned ethics content in nursing curricula is necessary to improve moral sensitivity and moral reasoning of students. (shrink)

The key theme is that the social construction of gender has its origins in the feminist theorists of this period. This paradigm is a key foundational element to both second and third wave feminist thought. It will focus on the five key scholars of the period: Komarovsky, de Beauvoir, Mead, Klein and Herschberger. As genomic science has evolved, so have policy and practice debates about how to describe and evaluate the ways in which genomic information is treated for individuals, institutions, and society. The term genetic exceptionalism, describing the concept that genetic information is special or unique, and specifically different from other kinds of medical information, has been utilized widely, but often counterproductively in these debates. We offer genomic contextualism as a new term to frame the characteristics of genomic science in the debates. (...) Using stasis theory to draw out the important connection between definitional issues and resulting policies, we argue that the framework of genomic contextualism is better suited to evaluating genomics and its policy-relevant features to arrive at more productive discussion and resolve policy debates. Previous research suggests that there are important gender disparities in the experience of leisure, but the issue of how mothers and fathers experience free time emotionally remains overlooked. The present study addressed this lacuna using the Experience Sampling Method and survey data from the 500 Family Study. Results showed that mothers and fathers spent the same amount of time on leisure activities. However, mothers had slightly less pure free time than fathers and were more likely to combine leisure with unpaid (...) work or spend time in leisure with children. Multilevel analyses showed that pure free time was associated with increased positive affect and engagement and decreased negative affect and stress, as was the combination of free time with unpaid work and personal care. These trends did not differ by gender. Adult leisure and free time with children were also beneficial to parents’ well-being. However, the relationship between free time with children and positive affect was stronger among fathers, whereas the association between adult leisure and engagement was stronger among mothers. These results suggest that mothers may feel more anxious about being criticized by others when engaging in leisure with their children, whereas spending time with adults alone may free them from the pressures of “good mothering.”. Social determinants of health are a subject of growing interest, yet criticisms have emerged about the way determinants are conceptualized in nursing. A tendency to focus on readily observable living conditions and measurable demographic characteristics can divert attention, it has been said, from the less visible underlying processes which shape social life and health. To illustrate how the analytic perspective determines what becomes visible or invisible as a “determinant” in health, this paper presents a case exemplar. Drawing from news reports (...) and research in real estate economics and urban policy analysis, it explores a single local infectious illness outbreak through a series of progressively more abstract units of inquiry, considering mechanisms of lending and debt financing, housing supply, property valuation, tax policy, change in the structure of the financial industry, and international patterns of migration and capital flow, among other factors, which contributed in various ways to creating unsafe living conditions. An analytic exercise calling attention to dynamism and complexity in social processes, the paper offers a political‐economy‐based approach that serves as a cautionary note against oversimplification in discussions of health causality. We propose a critique of normativism, defined as the idea that human thinking reflects a normative system against which it should be measured and judged. We analyze the methodological problems associated with normativism, proposing that it invites the controversial “is-ought” inference, much contested in the philosophical literature. This problem is triggered when there are competing normative accounts (the arbitration problem), as empirical evidence can help arbitrate between descriptive theories, but not between normative systems. Drawing on linguistics as a model, we (...) propose that a clear distinction between normative systems and competence theories is essential, arguing that equating them invites an “is-ought” inference: to wit, supporting normative “ought” theories with empirical “is” evidence. We analyze in detail two research programmes with normativist features – Oaksford and Chater’s rational analysis and Stanovich and West’s individual differences approach – demonstrating how, in each case, equating norm and competence leads to an is-ought inference. Normativism triggers a host of research biases in the psychology of reasoning and decision making: focusing on untrained participants and novel problems, analyzing psychological processes in terms of their normative correlates, and neglecting philosophically significant paradigms when they do not supply clear standards for normative judgement. For example, in a dual-process framework, normativism can lead to a fallacious “ought-is” inference, in which normative responses are taken as diagnostic of analytic reasoning. We propose that little can be gained from normativism that cannot be achieved by descriptivist computational-level analysis, illustrating our position with Hypothetical Thinking Theory and the theory of the suppositional conditional. We conclude that descriptivism is a viable option, and that theories of higher mental processing would be better off freed from normative considerations. (shrink)

The hypothesis that music is well suited to facilitate social bonding is highly consistent with social psychological research on the need to belong. We explore how music is uniquely placed to increase feelings of connections to large collectives by increasing collective effervescence, providing narratives, reminding one of others, and providing social surrogates.

We argue that imaginary worlds gain much of their appeal because they fulfill the fundamental need of human beings to feel connected to other humans. Immersion into story worlds provides a sense of social connection to the characters and groups represented in the world. By fulfilling the need to belong, imaginary worlds provide a buffer against rejection and loneliness.

Ask Cynthia Ozick to define a Jewish book and she launches into an extended defense of the integrity of the body of traditional Jewish writing: Torah, Talmud, liturgy, ethics, and philosophy. Ozick takes her Judaism seriously; its literature, in the broad sense, (...) for her, is didactic and dedicated to the promotion of virtue attained through study. She takes literature seriously too and strenuously defends fiction as an act of the "free imagination," with an equal emphasis on both elements of that phrase although, Ozick rightly argues, the imagination by definition is free. Thus, a Jewish book and Jewish fiction are contradictory. To different degrees, emphasis, and purpose, both are freighted by moral purpose.So are the various Ozicks! A traditional Jew, a Jewish American writer, a novelist, an essayist, a classical feminist—all describe Ozick who obviously belies all simple categories save one: writer. Yet even as a writer, Ozick is complex and conflicted, lamenting that she hasn't written enough fiction but claiming that it simply could not have been different.While Ozick may not be as prolific as she would like, she is a commanding presence in American fiction and belles lettres. Her latest collection of non-fiction work, Quarrel & Quandary, demonstrates her clear thinking, breadth of expertise, seriousness, and intelligence. Ranging from the autobiographical to the political, each essay is informed by history and none is light nor nostalgic. "Dostoyevsky's Unabomber" is perceptive and regrettably prescient in its depiction of the philosopher-murderer. In "The Posthumous Sublime," Ozick walks us through the "grieving that has been made beautiful" in the four stories of Sebald's The Emigrants. "A Drug Store Eden" is a warm and familiar but never sentimental portrait of her hard-working parents and her Uncle Ruby. The humor of "Lovesickness" and "How I Got Fired from My Summer Job" resonate for middle-class, studious, awkward young girls. In another essay, awe-inspired respect for Kafka is balanced by reverent awe of God, which is tempered by an uncomfortable acceptance of Job's acquiescence. Where is Job's "impious impatience" in the face of his renewed prosperity and health?, she asks. Where is "the father's bitter grief over the loss" of his first family? In quintessential [End Page 456] Ozick, she acknowledges the staying power of the ancient sages who argued God's majesty at the same time reserving her right to "philosophic doubt," especially in light of the Shoah.Two essays in this volume are exceptional in their insight and influence. "Who Owns Anne Frank?" should put to rest the sentimentalization of Anne Frank and her diary. It should stop the shameless marketing of the Holocaust—the catastrophe that marked a century—as yet another genocide. Ozick's relentless focus on the reality of Nazi evil and its consequent waste of human lives and potential should abort further distortions about Anne's short life, but it probably won't. The diary has been "bowdlerized, distorted, transmuted, traduced, reduced... infantilized, Americanized, homogenized, sentimentalized, falsified, kitschified, and, in fact, blatantly and arrogantly denied." It has "nonsensically" been described as cheerful and optimistic, even in the face of the fact that Anne, along with other members of the attic community, were targeted for murder and that, one month before liberation, Anne and her sister Margot died as prisoners in the unimaginable horror of Bergen Belsen, a camp so lice-ridden and disease-infested that it was burned to ashes by the British rather than cleansed, disinfected, or rehabilitated.Ozick tracks the controversy centered on the transformation of Anne's diary into the Americanized, christianized, perky Junior Miss that American audiences would accept, including the fabrication of lines that rendered the targeting of the Jews as one victim among many: "We're not the only people that've had to suffer" instead of Anne's more thoughtful and complex—and potentially problematic—"In the eyes of the world, we're doomed.... Alfred Russell Wallace’s The Malay Archipelago, published in 1869, is a classic text in natural history and the theory of evolution. Amidst heroic hunting narratives and picturesque descriptions of local fauna and flora, stands out a curious episode in which Wallace describes adopting a baby orangutan, whose mother he had killed. Wallace, a British naturalist and collector, cultivated an affectionate relationship with the orphaned orangutan, often referring to her as his “baby.” This paper examines how the orangutan was transformed from (...) being a moving target for museum display, to a beloved companion but also a scientific specimen. In this process, Wallace redesigned his colonial bungalow to a space that combined domestic settings with engineered nature-like environments, a familiar construction in later primate research. I use Wallace’s adoption episode to discuss how affect and care were interwoven into the exploitative relations of British naturalists and physiologists and the animals they studied. The account of Wallace’s idiosyncratic relationship with the orangutan is augmented with additional documentation of the close relationships of scientists with research animals, staged as familial kinship. The emergence of the “laboratory pet” demonstrates how the production of knowledge, the sharing of households, and human-animal emotional ties were interwoven in early biomedical research. (shrink)

The orthodox approach to time states it to be a continuum. In this paper I aim to show that the growing block model poses a unique problem to the continuity of time, on account of it being a hybrid A-B-theory. Tension lies in the fact that a continuous B-theoretical block is built through the A-theoretical becoming of instantaneous slices of present. First, I show that a continuous growing block necessitates a present with zero temporal duration; second, I show that such (...) notion of present rules out some widely accepted B-theoretical solutions to the problem of the continuum, while its commitment to the B-theory rules out some of the A-theoretical ones. Normativism, the approach that judges human rationality by comparison against normative standards, has recently come under intensive criticism as unsuitable for psychological enquiry, and it has been suggested that it should be replaced with a descriptivist paradigm. My goal in this paper is to outline and defend a meta-theoretical framework of such a paradigm, grounded rationality, based on the related principles of descriptivism and (moderate) epistemic relativism. Bounded rationality takes into account universal biological and cognitive limitations on human rationality. Grounded (...) rationality accepts universal constraints but adds cognitive variability: Within-individual variability (dual processing), and individual as well as cultural differences. I discuss the implications of grounded rationality to dual processing, proposing that investing limited cognitive resources in analytic processing might be less instrumentally rational for individuals with low cognitive ability. In recent years, the psychology of reasoning has been undergoing a paradigm shift, with general Bayesian, probabilistic approaches replacing the older, much more restricted binary logic paradigm. At the same time, dual processing theories have been gaining influence. We argue that these developments should be integrated and moreover that such integration is already underway. The new reasoning paradigm should be grounded in dual processing for its algorithmic level of analysis just as it uses Bayesian theory for its computational level of (...) analysis. Moreover, we propose that, within the new paradigm, these levels of analysis reflect on each other. Bayesianism suggests a specific theoretical understanding of dual processing. Just as importantly, the duality in processing carries over to duality in function; although both types of processes compute degrees of belief, they generate different functions. (shrink)

In previous published research (“Conditionals and Inferential Connections: A Hypothetical Inferential Theory,” Cognitive Psychology, 2018), we investigated experimentally what role the presence and strength of an inferential connection between a conditional’s antecedent and consequent plays in how people process that conditional. Our analysis showed the strength of that connection to be strongly predictive of whether participants evaluated the conditional as true, false, or neither true nor false. In this article, we re-analyse the data from our previous research, now focussing on (...) the semantics of conditionals rather than on how they are processed. Specifically, we use those data to compare the main extant semantics with each other and with inferentialism, a semantics according to which the truth of a conditional requires the presence of an inferential connection between the conditional’s component parts. Two Experiments demonstrate the existence of a “collapse illusion”, in which reasoners evaluate Truthteller-type propositions (“I am telling the truth”) as if they were simply true, whereas Liar-type propositions (“I am lying”) tend to be evaluated as neither true nor false. The second Experiment also demonstrates an individual differences pattern, in which shallow reasoners are more susceptible to the illusion. The collapse illusion is congruent with philosophical semantic truth theories such as Kripke's (1975), and with hypothetical thinking theory's principle of (...) satisficing, but can only be partially accounted for by the model theory principle of truth. Pragmatic effects related to sentence cohesion further reinforce hypothetical thinking theory interpretation of the data, although the illusion and cohesion data could also be accounted for within a modified mental model theory. The public rejection of scientific claims is widely recognized by scientific and governmental institutions to be threatening to modern democratic societies. Intense conflict between science and the public over diverse health and environmental issues have invited speculation by concerned officials regarding both the source of and the solution to the problem of public resistance towards scientific and policy positions on such hot-button issues as global warming, genetically modified crops, environmental toxins, and nuclear waste disposal. Curiosity and creativity are central pillars of human growth and invention. While they have been studied extensively in isolation, the relationship between them has not yet been established. We propose that curiosity and creativity both emanate from the same mechanism of novelty-seeking. We first present a synthesis showing that curiosity and creativity are affected similarly by a number of key cognitive faculties such as memory, cognitive control, attention, and reward. We then review empirical evidence from neuroscience research, indicating that the (...) same brain regions are involved in both curiosity and creativity, focusing on the interplay between three major brain networks: the default-mode network, the salience network, and the executive control network. After substantiating the link between curiosity and creativity, we propose a novelty-seeking model (NSM) that underlies them both and suggest that the manifestation of the NSM is governed by one's state of mind (SoM). The evidence-based medicine (EBM) movement is touted as a new paradigm in medical education and practice, a description that carries with it an enthusiasm for science that has not been seen since logical positivism flourished (circa 1920–1950). At the same time, the term ‘‘evidence-based medicine’’ has a ring of obviousness to it, as few physicians, one suspects, would claim that they do not attempt to base their clinical decision-making on available evidence. However, the apparent obviousness of EBM can and should (...) be challenged on the grounds of how ‘evidence’ has been problematised in the philosophy of science. EBM enthusiasm, it follows, ought to be tempered. The post-positivist, feminist, and phenomenological philosophies of science that are examined in this paper contest the seemingly unproblematic nature of evidence that underlies EBM by emphasizing different features of the social nature of science. The appeal to the authority of evidence that characterizes evidence-based practices does not increase objectivity but rather obscures the subjective elements that inescapably enter all forms of human inquiry. The seeming common sense of EBM only occurs because of its assumed removal from the social context of medical practice. In the current age where the institutional power of medicine is suspect, a model that represents biomedicine as politically disinterested or merely scientific should give pause. (shrink)
